The Health Information Department at SJH is staffed with nationally accredited specialists that are responsible for collecting, recording, reviewing and managing your health information.

Our practitioners proudly support the Canadian Institute for Health Information (CIHI) by collecting and submitting data to accelerate improvements in healthcare. Statistics are completed at a local, provincial, and national level.

We comply with The Health Information Protection Act (HIPA) to ensure your health information is kept safe and confidential. Health records can be physical, electronic, or a hybrid format. To request information from your own physical health record please fill out the form located at Health Information Services (Health Records) | SaskHealthAuthority. To access your health information electronically please refer to https://www.ehealthsask.ca/MySaskHealthRecord/MySaskHealthRecord . We do our best to fulfill requests promptly, but please be advised that our staff have 30 days to complete all requests.
